Recent Client Praise

During this pandemic, over 13,000 more Alzheimer’s and other dementia patients have died than expected from these cognitive disorders, and it’s not the virus. Those with any of the forms of dementia living in facilities designed to protect them from the deadly virus have died, not from COVID, but from isolation. The number of COVID deaths has broken through the 200,000 mark this week, and we all know that older adults are much more vulnerable to the virus than younger Americans. Senior communities, in particular, have been extraordinarily careful about not allowing family members from the outside to get near their older loved ones in order to protect residents from infection. But doctors have reported a surprising rise in the numbers of falls, pulmonary infections, sepsis and depression among dementia patients during these past six months among patients who had been quite stable and otherwise healthy for years prior to the pandemic. An option to explore with your loved one’s senior community is to seek permission to hire a private duty caregiver through an agency who can attend your loved one. Many communities are overworked and understaffed in the present pandemic/quarantine, and added personal attention will benefit your loved one. The Alzheimer’s Foundation of America (AFA) has some recommendations for families and caregivers of someone who has been diagnosed with Alzheimer’s or other dementia caring at home:
1. Dementia patients have sensitivity to water, so sing a couple verses of “Happy Birthday” as you help them wash as a pleasant distraction;
2. Be careful to be a calm model, because a dementia patient will be very attuned to what you say and your body language;
3. Since washing is critical and they may resist washing, talk in a soothing tone and consider a fragranced soap that they may like;
4. As an alternative for cleaning hands use an alcohol-based hand sanitizer – 60%+ alcohol according to CDC;
5. Encourage consistent hydration to bolster their immune systems, but also be prepared to assist with more trips to the restroom;
6. Have visual and tactile diversions on hand like photo albums, lively musical movies or just sorting the socks from the week’s laundry.
Everything about this pandemic is unprecedented, a word that I personally have used more times in the past six months that all of my years leading up to it. So, this is the latest learning in response to the scientists who are paying attention to the impact of COVID. We will keep you informed as we learn along with everyone else on this “unprecedented” journey. You know what makes a real difference between grade school children and adults over the age of forty? We could go down an entire list of physiological and intellectual and stature differences, but there is one you may not have considered, because it’s in your head. Ask most kids in primary school how old they are and you’ll get a fair bit of “I’m going to be ## in whatever month their next birthdays are. Ask someone over forty a similar question as a group of Danish researchers did of 1,470 study subjects and it flips pretty dramatically the other direction. On average, these researchers got answers that were 20 percent younger that the subjects’ actual ages. Across all of these study subjects the age deflation was a fairly consistent 20 percent difference which led the researchers, and probably you as well, to ask, “Why a consistent twenty percent?” What age do you think yourself to be in your head? I attended a seminar a number of years ago, and one of the lectures was from a recently-retired university professor who introduced his session with a simple rule: “You have to get older, but you don’t have to act your age.” He promptly plopped a beanie on his head and proceeded to elaborate. I spoke with him on our break and asked him what motivated him to retire from his tenured position and his university and take his subject matter to other venues. I did not then know to ask him how old he thought himself to be, but his story suggests that he wanted to put a brake on aging. His best friend on the faculty had died in the past year which gave him pause to take stock and reroute his life. Or, did he refuse to get to be “that age” but changing lanes to a track that felt more vital, younger? This is not an uncommon story line for “aging adults,” although the details of each person’s story will be different from the others. Some of this phenomenon may also be that our society tends to discount what an older adult can bring to the table. Ask anyone who still wishes to stay on their career path into their sixties or seventies…until they report to that new 35 year old manager. Our society and most western societies tend to attach less value to older workers’ wisdoms. Yet, Asian cultures celebrate age for its place within a hierarchy where chronological age is correlated with prestige. Their brain age to chronological age gap is also smaller and smaller yet is the gap between how old an individual is and how old they feel in their minds in Africa. These latter also tend to be a bit more culturally homogeneous and socioeconomically more homogenous as well. All of this does underscore that age divide, but it should not stop you from enjoying the gathering for your next school reunion or the music they play that “takes you back” to your youth. At the end of the day we are all an accumulation of countless experiences across our years, and you should feel to travel back to whenever you want. To answer the title’s question, what’s in your head may be worth revisiting whenever you choose. Enjoy your trip! Senior Home Safety
Having a safe home is a large part of senior health that is often overlooked until it’s too late to prevent an accident and serious injury. According to the National Institutes of Health, six out of ten falls happen in the home where we feel safe and secure. Our elder care management plans place an emphasis of senior home safety for just that reason. Small details like dim lighting, loose carpeting, hard to reach items or an uneven driveway, become safety hazards for aging adults.
Common Safety Hazards
As we grow older, we become more limited physically. Eyesight, strength and agility deteriorate, and we need to be more careful with our bodies as years progress. Addressing physical limitations by making changes around the home before an accident happens, can keep an older adult safe and comfortable at home. Be aware of clutter on the floor, inconveniently placed furniture, slippery or uneven flooring and area rugs that could cause a problem. Bathrooms and kitchens are often the most hazardous rooms. Add extra railings for support in the bathroom and on the stairs, improve the lighting in dim areas and place items at an easy to reach height to keep your older family member safe.

May is Mental Health Awareness month, and that makes it a good time to talk about America’s Drug epidemic…an epidemic that affects older adults every bit as much as it does teens. It also is a good time to start a conversation with that older loved one in your orbit. Mental health is not just an issue of seniors, but you might be surprised to know that prescription drug misuse disproportionately affects Americans 65 and older. And it is part of an growing trend. In 1988 about one in three Americans had been prescribed three or more medications, but that has grown to well over half the 65+ group now having three or more meds in their bathroom cabinet. And that does not include the rising rate of alcohol abuse among seniors in America. Why the increase? When you talk with public health officials they cite an increasingly fragmented health care delivery system where all of a patient’s physicians may not be communicating with one another. So, the number of prescriptions will rise as a function of the aging complaints and the number of physicians a patient sees. Older patients are receiving an escalating number of anti-depressants, anti-anxiety meds and prescriptions for sleeplessness. And older patients just live long enough to have more pain and more medical issues, and that begins to explain the escalating number who receive prescriptions for opioids…it is not just “take two aspirin and call me in the morning” any longer. And aging has a way of exposing an individual to more of life’s losses that simply take an emotional toll: • Retirement is not always freedom – for many it is an emotional loss • Older adults also can outlive spouses and friends – another emotional loss • Some older adults live in fear of outlasting their money – an emotional strain • Surgery or other life changing medical episodes – an emotional strain • And the list goes on… All of the above can result in that conversation with a physician who attempts to relieve some of the physical or emotional pain medicinally. Or an older adult can self-medicate with alcohol. So, if you are a caregiver to an older loved one or have someone special in your circle, take time for a conversation. You may be surprised by what you learn. You may also find that you can be part of a mentally healthy solution to what may be affecting that individual. Starting a conversation is a great way toward becoming part of the cure. We all know that Boomers - who began reaching age 65 just two years ago - are the largest segment of the U.S. population. Aging boomers are expected to total about 71.5 million by the year 2030, but what you may not know is that as many as one in ten of the Silver Tsunami will be lesbian, gay, bisexual or transgender (LGBT). This is important, because the generations that preceded them who were LGBT simply accommodated the "closet" when they aged. Never mind the ageism and racism we still find in some quarters, a lot of the institutional providers for seniors may not yet have embraced a welcoming culture for people of alternative gender preferences. As a geriatric care manager, I see this as a perfect storm with three fronts, a storm which has the potential to create one of the largest threats to aging well - social isolationism. If you are a caregiver to an older LGBT partner or loved one, you will need to be alert to the potential challenges. For instance, there are likely to be limited or no programs for alternative lifestyle older individuals in rural or suburban areas. And the gay male experience of aging is exaggerated among men where there is a more pronounced premium on being attractive or desirable. Generally speaking being 40+ among gay men can be seen as equivalent to being 80+ in the straight community in terms of being marginalized. The social isolation that is a consequence of "premature aging" and marginalization also increases the gay population's risk for depression, substance abuse, low self-esteem or even suicide. The rate of suicide among gay men is about six times that of straight men. LBT women may have it even worse. More than one-third, according to recent surveys, say that they have not divulged their sexual preferences to their health care providers. They also are less likely than gay and bisexual men to have purchased long term care or life insurance. About one in five LBT women are caring for an aging partner, but they also feel less prepared financially than gay men for retiring. Put all these factors together, and the LGBT community is less inclined to seek attention for health issues for fear of hostile or judgmental providers. Poor health care translates into even earlier institutionalization in long term care facilities. Also, it is ironic that in national surveys that nearly three in four LGBT individuals anticipate that they will be caregivers to another person, but one in five are unsure they will be cared for themselves when the need arises. Adding to this potential caregiver shortfall, many gay couples simply do not have conventional family to care for them. If you are a caregiver to an LGBT elder, encourage the following: • They have a Last Will and Testament that is really according to their "will" o With clear advanced directives/living will • Their partner/spouse has Durable Power of Attorney (health and finance) • Informal caregiving arrangements are clear and followed • Priority rights for hospital visitation are followed • There is Joint Ownership of Property with Rights of Survivorship And for us all: Recognize that all humans deserve compassionate care which respects their personhood and their definition of family and their wishes. What do hearing aids, exercise, support groups and travel have in common? They all are ways that individuals suffering from Alzheimer’s Disease, specifically, or dementia, generally, can fight these diseases’ progression. Even more important, they help to fight the personal isolation that accompanies these frightening conditions.

We often associate dementias with older adults and the stereotypic confusion that can characterize the individuals with the diagnoses. But it is not just a condition of older adults as the recent public disclosure of The University of Tennessee women’s basketball coach, Pat Summit, highlights. Ms. Summit is just 59 as she goes public with her diagnosis, and it helps others to understand that it can happen as early as 30 and 40 years of age. Younger onset Alzheimer’s Disease technically speaking is the manifestation affecting those under age 65, and there are 250,000 new cases in America every year. Yet, there are ways to fight the downward spiral of dementia.

Hearing aids may seem an unlikely fix for dementia. Yet, it is simplybecause hearing aids enhance the clarity of communication and therefore the connection between dementia patients and others that they help alleviate the isolation and bolster memory. When dementia patients can hear more clearly, that connection also helps calm the anxiety that they may otherwise feel when living in a world that is confusing. Hearing aids cannot “cure” dementia, but they can help a patient navigate more confidently.

Exercise can be a helpful boost as well as it improves the blood flow and releases calming endorphin into the bloodstream. For a variety of medical conditions, health care providers often talk about the role that “performance status” plays in a patient’s ability to cope. Being more fit helps dementia patients cope just as being fit also helps cancer patients while they fight a tumor or surgical patients as they recover from a procedure.

I had mentioned the importance of being connected for a person with dementia of whatever sort. Support groups become the new circle of friends who can truly understand what it is like. This is common across a whole host of medical predicaments from grieving widows and family to returning wounded warriors or recovering cardiac or orthopaedic surgery patients. Somebody else who has been there understands what co-workers, friends or even family cannot quite fathom.

Travel is a bit of all these. It is a way to have fresh new experiences that challenge a patient cognitively. When a person experiences what should be familiar settings like home as frighteningly unfamiliar, it can be reassuring to share travel experiences to strange and different places that – well – everyone will find strange and different as well.

It’s not just about being lonely. Isolation may be a bigger senior health issue than even dementia – never mind that research has shown a clinical link between these two. I had a client a few years back who responded to a question from an attendee at her 100^th birthday about how it feels to be 100 by saying simply, “Not so much peer pressure…” Now, this individual was surrounded by friends and family, but what she highlighted was the attrition of social connection that accompanies aging in most individuals. As one ages, one loses friends and loved ones and one also loses the ability to physically get out and about. As one becomes increasingly isolated, there are mental, physical and emotional correlates of the loneliness:

Research published in 2012 showed a clear relationship between social isolation and death among adults 52 years of age and older. One cannot say that isolation kills people, but one’s chances of dying from any cause are greater if living alone.

Research from the University of Chicago has found a clear link between levels of loneliness and poor cognitive function and even dementia risk. In other research reported in 2016 older patients with the amyloid markers of Alzheimer’s were 7.5 times as likely as those without the markers to report feeling isolated or lonely.

The National Center on Elder Abuse adds to the bad news by reporting that socially isolated seniors have higher rates of physical or financial abuse. Are they easier targets for predators or do predatory caregivers isolate them to take advantage?

Isolated seniors also report more of just about every chronic medical condition from COPD to heart disease, poor mobility and, of course, depression. Loneliness is even linked to high blood pressure. This, after taking into account gender, ethnicity and other factors.

Most of these can give rise to the proverbial chicken and egg arguments about loneliness as a cause of the problems or the problems themselves giving rise to loneliness. So, let’s turn it around and look at what you can do if you are one of the circle of caregivers for an older loved one. Loss of a spouse leads to isolation. Be present in the weeks and months (not just days) following such a loss to help an older loved one re-integrate into a new social support system. Transportation challenges can lead to social isolation. If you have a loved one without transportation, look for alternatives like para-vans, carpooling and volunteers. Caregivers are at risk for social isolation. Find respite care resources, lay volunteers from religious organizations and home care options to give these caregivers a break. And don’t forget the isolation of that caregiver’s care receiver. Loneliness can be contagious. Volunteering can go the other direction also. Encourage your older loved one to engage in community activities, take a class or seminar, join a club, etc. And appreciate that with these added social activities you are helping to prevent a slide into unhealthy behaviors for your older loved one, such as smoking, drinking or just poor diet. Now is the best time of all to engage an older loved one in new activities. It is, after all, the New Year. And it is the season of long and cold nights. Encourage activities that can be part of the good habits of the new year. When I say there is no ‘D’ in dementia, it is not that I am Deficient in spelling. Medical research has been accumulating that people with low levels of vitamin D in their bloodstreams are more likely to experience cognitive decline and even develop Alzheimer’s Disease. When I say cognitive issues, it is not all aspects of brain function. The declines associated with vitamin D deficiency are focused on episodic memory loss and deterioration of executive function. Researchers from University of California, Davis and Rutgers University studied 382 adults whose average age was 75.5 years old. These adults received cognitive assessments which categorized them as having dementia (17.5%), having mild cognitive impairment (32.7%) or as cognitively normal (49.5%). Vitamin D levels were lowest for those diagnosed with dementia, but there was only very nominal difference in vitamin D levels for those who were mildly impaired and those who were classified as cognitively normal. All of these adults were studied for nearly five years and then assessed again. During that time the magnitudes of decline in episodic memory and executive function were greater in adults who had been diagnosed as vitamin D deficient or vitamin D-insufficient. The investigators found that regardless of their baseline cognitive assessment, their ethnicity or a range of other risk factors, those who had been vitamin D deficient at the beginning of the five year period were at greater risk of developing Alzheimer’s Disease. Scientists continue to study the association between vitamin D levels and dementia. While there are no studies that suggest taking vitamin D supplements can prevent Alzheimer’s, doctors in practice are not waiting. They are prescribing D for their older patients, especially those who are already at low blood levels of the vitamin. We who live in northern regions should be particularly on top of getting enough sun to activate our natural vitamin D, consuming more dairy products and green leafy vegetables and augmenting our diets with vitamin supplements. If you were diagnosed with dementia, would you want to be told? If you said “yes,” you are in agreement with nine of ten Americans in nearly two dozen national surveys on the subject. Even among patients actually diagnosed with some form of dementia, 85% said that they wanted to know their diagnosis. Yet, according to the Alzheimer’s Association fewer than 50% of Alzheimer’s patients or their caregivers actually have been told by their healthcare providers. Why the disconnect? I have written in the past about people’s greatest fears as they age, and it is not that they may (will) die; it is losing their cognitive abilities. A fair number of healthcare providers reportedly withhold the dementia diagnosis from their patients and the patients’ families because they are concerned about the stigma that knowing may represent. Others are concerned about the impact a misdiagnosis might have or they just do not know how to communicate with these patients or help the family learn to cope with the diagnosis. Some providers also write this off as just another part of “normal aging” or ironically they “forget” to mention these cognitive symptoms. Despite all the logic or the excuses, there remains an ethical obligation to disclose a dementia diagnosis and to help the patient and family confront the issues head on. There is a general appreciation that dementia is presently incurable, but knowledge of the diagnosis early can help families to decide on treatments as well as a future plan of care. There are currently five FDA-approved treatments for Alzheimer’s Disease … the acetylcholinesterase inhibitors like Aricept and memantine, Namenda. None of these can prevent or reverse the disease, and none of these is indicated for other dementias beyond Alzheimer’s. But they can slow or eliminate the build-up of beta-amyloid or tau proteins. In a recent posting I also talked about dementia being type 3 diabetes, so medications that can limit the inflammation in the brain or insulin resistance also can have some benefit for patients. And there are therapies in development that show some promise as well. While all states have some level of mandatory reporting of abuse on the part of healthcare providers, only a handful require dementia disclosure. More require reporting epilepsy as a traffic safety issue than require dementia. But it is more than a traffic safety matter; disclosure is one more step toward helping the caregivers to dementia sufferers get training and help in their day to day activities. As recently as 2012, it has been reported that more than 15 million Americans provided in excess of 17 billion hours or unpaid care to loved ones with dementia. If you are a caregiver to someone showing signs of cognitive decline, speak with your loved one first and then accompany them to their next physician’s visit. Speak with the physician about your concerns. Most likely you will need a referral to a neurologist who will put in an order for a brain scan. If you insist on a candid conversation, not only will you know what the actual diagnosis is, but you will be directed to a host of community resources to help everyone manage your care receiver’s condition. Even among those older adults who dodged COVID, “gymnesia” may be a lingering effect. Please let me explain…

What business has more facilities than McDonald’s, Dunkin’ Donuts and Taco Bell combined, and saw almost universal closings during the pandemic? And what business saw far fewer clients returning than McDonald’s, Dunkin’ Donuts and Taco Bell combined as the pandemic restrictions have begun to relax? The answer may not come as much of surprise since even I have been holding back from going back to the fitness center for my daily caloric burn. There are over 32,000 fitness centers across America, and a lot of them continue to remain shuttered in this phase of the pandemic that some are beginning to call the “post-pandemic.” Almost all of these facilities have gone to extraordinary lengths to make their establishments cleaner (air and contact surfaces) and safer than ever before, but some folks, especially older clients, are hesitant to get back into their routines. It’s time to get back into shape, and here are some tips for the older adults in your circle: Safety check. You can call or email any of these facilities, and when you do be prepared with some important questions. How often and how extensively do they clean equipment and locker rooms? Ask about how they have upgraded their ventilation systems. Then go on-line and see how they compare to what is recommended by public health experts. Getting there. The average person travels 2.6 miles for their fast food favorite and 3.8 miles for their groceries. No surprise, then, that most people will travel less that four miles to work out. Go ahead and Google: “fitness center near me,” and you’ll probably be surprised how many spas, gyms or workout facilities are within four miles. Walk through. Of course, you won’t sign up without a visit to see what a facility looks like close-up…and yes, walk through their locker rooms as well. Really key, however, is when to schedule your visit. Set a time that matches when you like to work out so that you can see exactly what you will experience if you sign the contract. Partner Up. This is not about safety and the importance to not go to the facility alone. Find a friend or encourage your spouse who will buddy up with you for your regular workouts. It’s nice to use the guilt of not wanting to leave our work out buddy all alone to get us to make the trip each day that we have committed. How much. There are some spectacular palaces to body fitness, and they may be right for you. Yet, if you’re just a yoga type or a Pilates junky, a smaller venue is more than adequate. Some of you may still want to work from the safety of home; then, consider a trainer, even if it’s only once a week to hit the rest button to keep you at it. I personally like the machines that tell me how many calories I burn with each workout, and my phone has an app that helps as well. It’s a built-in reward for all the sweat, and it does feel a lot better.
